Lakini kufunguliwa zaidi vituo vya kupokelea wagonjwa hao wa kipindupindu, na uamuzi wa serikali kusitisha kwa muda shughuli za baadhi ya skuli za umma na za binafsi ni dalili ya wazi juu ya ukubwa wa mripuko huo.
Mripuko wa kipindupindu si tukio jipya visiwani Zanzibar, Tanzania au katika nchi za ukanda wa joto. Mripuko mkubwa zaidi kuvamia Zanzibar ulikuwa mwaka 1978 ambao ulileta maafa makubwa ya vifo kwa mamia kama si maelfu kamwe. Kuanzia hapo Zanzibar imekuwa ikikabiliwa na mripuko wa kipindupindu kila mwaka kama 1992, 1994, 1997, 1998 na 2000. Mingi ya miripuko hii huvikumba Visiwa vya Unguja na Pemba. Na mara nyingi miripuko hii husababishwa moja kwa moja na msimu wa mvua kuanzia Machi hadi Juni.
Pamoja na ukweli wa uwepo wa kipindupindu marakwa mara Zanzibar, na pamoja na kuwepo mapendekezo mengi ya ndani na ya kimataifa ili kukabiliana na janga hili, inaonekana bado wanasiasa wamekosa utashi na utayari katika kukinga mripuko wake.
Uchambuzi mara zote unaonesha kuwa maeneo yanayokumbwa na janga la kipindupindu yanakabiliwa na hali duni katika matumizi ya vyoo na kukabiliwa na uhaba mkubwa wa maji safi na salama, hali inayofanya jamii za maeneo hayo kuwa katika mazingira hatarishi zaidi. Uchunguzi wa kuchambua usalama wa maji uliofanywa na Chuo Kikuu cha Milano (Italy) Zanzibar ndani ya mwezi Machi mpaka Juni 2006, uligundua kuwepo vimelea vya kipindupindu katika vyanzo vya maji 41 katika vyanzo 50. Maji yaliyochukuliwa kutoka vyanzo mbalimbali kutoka katika wilaya tatu tofauti. (‘Global Task Force on Cholera Control’, Mission Report, Zanzibar July 2006)
Zanzibar kama mataifa mengine machanga imegubikwa na rushwa, ufisadi na usimamizi mbovu katika taasisi ya mipango miji, mazingira na usambazaji wa maji safi na salama. Mambo ambayo khatima yake hupelekea mripuko wa kipindupindu na aina nyengine za maradhi.
